The reporting box accepts any reports regarding suspected violations of animal welfare operations or behaviors. Any person may participate and submit reports anonymously. The rights and interests of the reporter will be fully protected, and the reporter shall not be subjected to any retaliation.

公司员工活公司到访人员有权利以各种方式向IACUC汇报任何违背“善待动物”原则的现象，并监督IACUC委员的工作；
Any member in the institution may report to the IACUC if finding any violation of the principle of "Be kind to animals" in the management and use of experimental animals.
南京云桥璞瑞动物福利委员会鼓励汇报人以各种方式进行汇报。汇报人可以釆取匿名汇报的方式，特殊情况下，可直接口头向IACUC主席、委员或者兽医师进行汇报。汇报人的权益将完全受到保护，不会被惩处。
IACUC encourages reporters reporting in a variety of ways. The reporter may report anonymously or, under special circumstances, directly report orally to the IACUC chairman, members or veterinarian.The rights and interests of the reporter will be fully protected and will not be subject to any punishment.
须向IACUC汇报的事件
1损害动物健康和福利的情况，比如自然灾害、事故、故障等，从而导致了实际的动物损伤或死亡。
2进行动物相关的操作但未经IACUC审查和批准。
3对经IACUC批准的方案进行更改，但执行时未提前经过IACUC再次批准。
4进行动物操作超出了IACUC批准的有效期。
5不能按照Guide的要求为动物提供合适的空间。
6参与动物试验操作的人员没有经过合适的培训。
7不能在操作后监测动物状态，而导致无法确保动物福利。
8不能保存合适的动物相关记录（动物编号、用药史、饲养管理等）。
9不能在安乐死过程后确保动物的死亡。
10没有合适的动物使用责任人员执行兽医医嘱（比如动物治疗）。
1.Incidents that must be reported to IACUC
1. Damage to animal health and welfare, such as natural disasters, accidents, faults, etc., resulting in actual animal injury or death.
2. Animal-related operations are conducted but not reviewed or approved by IACUC.
3. Make changes to a protocol that has been approved by IACUC but not re-approved by IACUC in advance at execution.
4. Animal operations exceed the validity period approved by the IACUC.
5. Unable to provide suitable space for animals according to the requirements of the Guide.
6. People involved in the operation of animal tests are not properly trained.
7. Unable to monitor animal status after operation, resulting in failure to ensure animal welfare.
8. Unable to keep appropriate animal records (animal number, medication history, feeding management, etc.).
9. Unable to ensure the death of animals after the euthanasia process.
10. No appropriate heads in charge of animal use to carry out veterinary orders (e.g. animal treatment).
